ABSTRACT

Correlates of protection (CoP) are biological parameters that predict a certain level of protection against an infectious disease. Well-established correlates of protection facilitate the development and licensing of vaccines by assessing protective efficacy without the need to expose clinical trial participants to the infectious agent against which the vaccine aims to protect. Despite the fact that viruses have many features in common, correlates of protection can vary considerably amongst the same virus family and even amongst a same virus depending on the infection phase that is under consideration. Moreover, the complex interplay between the various immune cell populations that interact during infection and the high degree of genetic variation of certain pathogens, renders the identification of immune correlates of protection difficult. Some emerging and re-emerging viruses of high consequence for public health such as SARS-CoV-2, Nipah virus (NiV) and Ebola virus (EBOV) are especially challenging with regards to the identification of CoP since these pathogens have been shown to dysregulate the immune response during infection. Whereas, virus neutralising antibodies and polyfunctional T-cell responses have been shown to correlate with certain levels of protection against SARS-CoV-2, EBOV and NiV, other effector mechanisms of immunity play important roles in shaping the immune response against these pathogens, which in turn might serve as alternative correlates of protection. This review describes the different components of the adaptive and innate immune system that are activated during SARS-CoV-2, EBOV and NiV infections and that may contribute to protection and virus clearance. Overall, we highlight the immune signatures that are associated with protection against these pathogens in humans and could be used as CoP.

ABSTRACT

In this brief opinion piece, we highlight our studies characterizing adaptive SARS-CoV-2 immune responses in infection and vaccination, and the ability of SARS-CoV-2-specific T cells to recognize emerging variants of concern, and the role of pre-existing cross-reactive T cells. In the context of the debate on correlates of protection, the pandemic's progression in the past three years underlined the need to consider how different adaptive immune responses might differentially contribute to protection from SARS-CoV-2 infection versus COVID-19 disease. Lastly, we discuss how cross-reactive T cell responses may be useful in generating a broad adaptive immunity, recognizing different variants and viral families. Considering vaccines with broadly conserved antigens could improve preparedness for future infectious disease outbreaks.

ABSTRACT

Self-report measures partially explain consumers' purchasing choices, which are inextricably linked to cognitive, affective processes and implicit drives. These aspects, which occur outside of awareness and tacitly affect the way consumers make decisions, could be explored by exploiting neuroscientific technology. The study investigates implicit behavioural and neurovascular responses to emotionally arousing and high-engagement advertisements (COVID-19 content). High-engagement advertisements and control stimuli were shown in two experimental sessions that were counterbalanced across participants. During each session, hemodynamic variations were recorded with functional Near-Infrared Spectroscopy (fNIRS) of the prefrontal cortex (PFC), a neurophysiological marker for emotional processing. The implicit association task (IAT) was administered to investigate the implicit attitude. An increase in the concentration of oxygenated haemoglobin (O2Hb) was found for the high-engagement advertising when this category of stimuli was seen first. Specular results were found for deoxygenated haemoglobin (HHb) data. The IAT reported higher values for highly engaging stimuli. Increased activity within the PFC suggests that highly engaging content may be effective in generating emotional arousal and increasing attention when presented before other stimuli, which is consistent with the higher IAT scores, indicating more favourable implicit attitudes. This evidence suggests that the effectiveness of highly engaging advertising-related messages may be constrained by the order of advertisement administration.

ABSTRACT

We evaluated antibodies to the nucleocapsid protein of SARS-CoV-2 in a large cohort of blood donors in the United States who were recently infected with the virus. Antibodies to the nucleocapsid protein of SARS-CoV-2 indicate previous infection but are subject to waning, potentially affecting epidemiologic studies. We longitudinally evaluated a cohort of 19,323 blood donors who had evidence of recent infection by using a widely available serologic test to determine the dynamics of such waning. We analyzed overall signal-to-cutoff values for 48,330 donations (average 2.5 donations/person) that had an average observation period of 102 days. The observed peak signal-to-cutoff value varied widely, but the waning rate was consistent across the range, with a half-life of 122 days. Within the cohort, only 0.75% of persons became seronegative. Factors predictive of higher peak values and longer time to seroreversion included increasing age, male sex, higher body mass index, and non-Caucasian race.

ABSTRACT

COVID-19 working restrictions resulted in a location shift of white-collar workers into their home office (HO). Little is known about how the proportion of HO affects sitting and physical activity (PA) behaviour during working hours, and potential correlates are not well understood. A cross-sectional sample of currently working adults in HO (n = 575) completed questionnaires regarding HO proportion before and during the pandemic, work-related sitting and PA, and socioecological factors regarding HO sitting time. The reported HO proportion increased by 46.7 ± 40.6% during COVID-19 working restrictions. Workday sitting proportion correlates positively with HO proportion. The regression model identified gender, education level, working hours, working environment to stand during work, and transport and leisure PA as correlates of the workday sitting proportion in HO. This study provides findings that high HO proportions are related to higher work-related sitting times and adds insights into possible correlates of sitting time in HO.Practitioner summary: Working in the home office (HO) is a common characteristic of flexible work in white-collar workers. This study found that the workday sitting proportion correlates positively with HO proportion and identified correlates of the workday sitting in HO. Practitioners should consider the potential impacts of HO on work-related sedentary behaviour in future workplace health promotion.

ABSTRACT

A defined immune profile that predicts protection against a pathogen-of-interest, is referred to as a correlate of protection (CoP). A validated SARS-CoV-2 CoP has yet to be defined, however considerable insights have been provided by pre-clinical vaccine and animal rechallenge studies which have fewer associated limitations than equivalent studies in human vaccinees or convalescents, respectively. This literature review focuses on the advantages of the use of animal models for the definition of CoPs, with particular attention on their application in the search for SARS-CoV-2 CoPs. We address the conditions and interventions required for the identification and validation of a CoP, which are often only made possible with the use of appropriate in vivo models.

ABSTRACT

Political conflict is not a new phenomenon. Most political conflict is theoretical and nonviolent;however, when political violence does occur, the consequences can be detrimental to democracies. Recently, in the U.S., political violence in the form of extremism is predominantly rooted in far-right ideologies, such as racially and ethnically motivated violent extremism (REMVE) and anti-government anti-authority violent extremism (AGAAVE). In the last few years, the frequency and intensity of violence perpetrated by groups and individuals who hold these extreme ideologies have increased. The threat of one of the most consequential types of political violence, electoral violence, became real on January 6th, 2021. On the morning of January 6th, then-President Trump held the so-called "Save America" rally, near the U.S. Capitol, and circulated the false notion that the 2020 election was fraudulent. When it was over, and out of approximately 30,000 individuals who attended the rally and exercised their constitutional rights, a group of approximately 1,000 individuals headed to the U.S. Capitol. There, the group of insurrectionists stormed the building, aiming to stop the certification of the 2020 Presidential election. A year and a half later, hundreds of individuals have been charged with violent and nonviolent crimes for their participation in the attack. Of the individuals who chose to storm the U.S. Capitol, why did some commit electoral violence while others did not? The current study aims to better understand the correlates of electoral violence in the January 6th Capitol attack. Using a sample of n = 818 violent and nonviolent insurrectionists charged for their participation in the insurrection, several theoretically relevant correlates (i.e., psychological factors, structural factors, racial animosity, religious affiliation, conspiracy theories, extremist ideologies and ties to extremist groups, the role of the Internet and social media, criminal record, grievances about the COVID-19 pandemic measures, and political rhetoric) were examined using a mixed methods approach. The quantitative results suggested that criminal record and Internet use were significant predictors of engaging in electoral violence. The qualitative results (n = 20) corroborated those findings and highlighted the importance of racial animosity, extreme political ideologies, political rhetoric, and conspiracy theories. The implications of the current project include summarizing the possible correlates of electoral violence in the U.S., informing counter-extremism strategies and policies, and offering an avenue for future research on far-right violent extremism in the country. ABSTRACT

High and equitable COVID-19 vaccination coverage is important for pandemic control and prevention of health inequity. However, little is known about socioeconomic correlates of booster vaccination coverage. In this cross-sectional study of all Norwegian adults in the national vaccination program (N = 4,190,655), we use individual-level registry data to examine coverage by levels of household income and education of primary (≥2 doses) and booster (≥3 doses) vaccination against COVID-19. We stratify the analyses by age groups with different booster recommendations and report relative risk ratios (RR) for vaccination by 25 August 2022. In the 18-44 y group, individuals with highest vs. lowest education had 94% vs. 79% primary coverage (adjusted RR (adjRR) 1.15, 95%CI 1.14-1.15) and 67% vs. 38% booster coverage (adjRR 1.55, 95% CI 1.55-1.56), while individuals with highest vs. lowest income had 94% vs. 81% primary coverage (adjRR 1.10, 95%CI 1.10-1.10) and 60% vs. 43% booster coverage (adjRR 1.23, 95%CI 1.22-1.24). In the ≥45 y group, individuals with highest vs. lowest education had 96% vs. 92% primary coverage (adjRR 1.02, 95%CI 1.02-1.02) and 88% vs. 80% booster coverage (adjRR 1.09, 95%CI 1.09-1.09), while individuals with highest vs. lowest income had 98% vs. 82% primary coverage (adjRR 1.16, 95%CI 1.16-1.16) and 92% vs. 64% booster coverage (adjRR 1.33, 95%CI 1.33-1.34). In conclusion, we document large socioeconomic inequalities in COVID-19 vaccination coverage, especially for booster vaccination, even though all vaccination was free-of-charge. The results highlight the need to tailor information and to target underserved groups for booster vaccination.

ABSTRACT

Zaire ebolavirus (EBOV), Sudan ebolavirus (SUDV) and Marburg virus (MARV), are members of the Filoviridae family that can cause severe disease and death in humans and animals. The reemergence of Ebola, Sudan and Marburg virus disease highlight the need for continued availability of safe and effectives vaccines as well as development of new vaccines. While randomized controlled trials using disease endpoints provide the most robust assessment of vaccine effectiveness, challenges to this approach include the unpredictable size, location, occurrence and duration of filovirus disease outbreaks. Thus, other approaches to demonstrating vaccine effectiveness have been considered. These approaches are discussed using examples of preventive vaccines against other infectious diseases. In addition, this article proposes a clinical immunobridging strategy using licensed EBOV vaccines as comparators for demonstrating the effectiveness of filovirus vaccine candidates that are based on the same licensed vaccine platform technology.

ABSTRACT

Independent mobility (IM) is associated with children's physical activity and indicators of social, motor, and cognitive development. We surveyed Canadian parents of 7- to 12-year-olds (n = 2291) about social-ecological correlates of IM in the second wave of COVID-19 (December 2020). We used multi-variable linear regression models to identify correlates of children's IM. Our final model (R2 = 0.353) included four individual-, eight family-, two social environment- and two built environment-level variables. The correlates of boys' and girls' IM were similar. Our findings suggest that interventions to support children's IM in a pandemic context should target multiple levels of influence.

ABSTRACT

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) began churning out incredulous terror in December 2019. Within several months from its first detection in Wuhan, SARS-CoV-2 spread to the rest of the world through droplet infection, making it a pandemic situation and a healthcare emergency across the globe. The available treatment of COVID-19 was only symptomatic as the disease was new and no approved drug or vaccine was available. Another challenge with COVID-19 was the continuous mutation of the SARS-CoV-2 virus. Some repurposed drugs, such as hydroxychloroquine, chloroquine, and remdesivir, received emergency use authorization in various countries, but their clinical use is compromised with either severe and fatal adverse effects or nonavailability of sufficient clinical data. Molnupiravir was the first molecule approved for the treatment of COVID-19, followed by Paxlovid™, monoclonal antibodies (MAbs), and others. New molecules have variable therapeutic efficacy against different variants or strains of SARS-CoV-2, which require further investigations. The aim of this review is to provide in-depth information on new molecules and repurposed drugs with emphasis on their general description, mechanism of action (MOA), correlates of protection, dose and dosage form, route of administration, clinical trials, regulatory approval, and marketing authorizations.

ABSTRACT

Group 2B ß-coronaviruses (sarbecoviruses) have caused regional and global epidemics in modern history. Here, we evaluate the mechanisms of cross-sarbecovirus protective immunity, currently less clear yet important for pan-sarbecovirus vaccine development, using a panel of alphavirus-vectored vaccines covering bat to human strains. While vaccination does not prevent virus replication, it protects against lethal heterologous disease outcomes in both severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and clade 2 bat sarbecovirus challenge models. The spike vaccines tested primarily elicit a highly S1-specific homologous neutralizing antibody response with no detectable cross-virus neutralization. Rather, non-neutralizing antibody functions, mechanistically linked to FcgR4 and spike S2, mediate cross-protection in wild-type mice. Protection is lost in FcR knockout mice, further supporting a model for non-neutralizing, protective antibodies. These data highlight the importance of FcR-mediated cross-protective immune responses in universal pan-sarbecovirus vaccine designs.

ABSTRACT

Introduction with Problem Statement and Approach, Material and Methods, Results, Discussion, Conclusions Problem Statement. The lockdown against coronavirus 2019 pandemic has led to a decrease in physical activity (PA), but any study has analyzed PA' evolution during this transition following the social-ecological theoretical framework of human development. It suggests that PA engagement depends on the interaction of individual, interpersonal and environmental determinants. This model is essential to better understand PA engagement and to prevent future crises. Purpose. This study investigated the evolution of adolescents' PA with their protective and inhibitory barriers according to ecological model of PA engagement. Methods. A longitudinal follow-up of 808 French adolescents between before and during lockdown were questioned from PA and socio-demographic data. Wilcoxson signed rank test and Boruta forest algorithm were used to quantify the evolution of each PA intensity and to identify protective and inhibitory socio-ecological's determinants on PA behaviour. Results. Our results showed a significant decrease in the overall PA (-231 Mets.min.week-1, p<.05). More specifically, low PA decreased significantly (-480 Mets.min.week-1, p<.05), while moderate and vigorous PA has remained significantly stable during the lockdown (+0 Mets.min.week-1, p<.05). The three determinants of the socioecological framework are included in the model of evolution of each PA' intensity. Individual (PA initial profile, 58.8 median importance score) and environmental (garden: 10.42;housing: 7.93 median importance score) determinants are the two most important. Interpersonal determinants have not a significantly importance to explain PA' evolution. Conclusions. This study highlighted the complex and non-linear process of PA engagement at a critical period where the determinants are being disrupted. Socio-ecological tools are identified to rethink prevention strategies adapted to future crisis periods. Future interventions to promote PA should not target one determinant of the system but should consider the interactions of all socio-ecological variables. © 2023, Editura Universitatii din Pitesti. ABSTRACT

In vaccine research towards the prevention of infectious diseases, immune response biomarkers serve as an important tool for comparing and ranking vaccine candidates based on their immunogenicity and predicted protective effect. However, analyses of immune response outcomes can be complicated by differences across assays when immune response data are acquired from multiple groups/laboratories. Motivated by a real-world problem to accommodate the use of two different neutralization assays in COVID-19 vaccine trials, we propose methods based on left-censored multivariate normal model assuming common assay differences across settings, to adjust for differences between assays with respect to measurement error and the lower limit of detection. Our proposed methods integrate external paired-sample data with bridging assumptions to achieve two objectives, both using pooled data acquired from different assays: (i) comparing immunogenicity between vaccine regimens, and (ii) evaluating correlates of risk. In simulation studies, for the first objective, our method leads to unbiased calibrated assay mean with good coverage of bootstrap confidence interval, as well as valid test for immunogenicity comparison, while the alternative method assuming constant calibration model between assays leads to biased estimate of assay mean with undercoverage problem and invalid test with inflated type-I error; for the second objective, in the presence of noticeable left-censoring rate, our proposed method can drastically outperform the existing method that ignores left-censoring, in terms of reduced bias and improved precision. We apply the proposed methods to SARS-CoV-2 spike-pseudotyped virus neutralization assay data generated in vaccine and convalescent samples by two different laboratories.

ABSTRACT

Several studies have shown that neutralizing antibody levels correlate with immune protection from COVID-19 and have estimated the relationship between neutralizing antibodies and protection. However, results of these studies vary in terms of estimates of the level of neutralizing antibodies required for protection. By normalizing antibody titers, we found that study results converge on a consistent relationship between antibody levels and protection from COVID-19. This finding can be useful for planning future vaccine use, determining population immunity, and reducing the global effects of the COVID-19 pandemic.

Introduction: Coronavirus Disease-2019 (COVID-19) pandemic exposed the health workforce to an unprecedented occupational hazard. While taking care of patients they always had to be conscious simultaneously for safeguarding themselves and their family members against the highly infectious virus. In West Bengal, cases were first reported in the last week of March-2020 and reached the peak around October-2020 in the first wave, once the lockdown was lifted. During the initial months, the staggering number of cases, prevailing uncertainty over case management, and untimely demise of colleagues and relatives, took their toll on the physical and mental health of doctors, paramedics, or support staff, both in the government and private sectors. Aim(s): To measure perceived stress, resilience and psychological well-being of healthcare providers using standard psychometric tools. Material(s) and Method(s): This was a cross-sectional observational study carried out among healthcare workers in hospitals located in West Bengal, India. A self-administered questionnaire was circulated through a digital platform between June-November 2020. The questionnaire was designed using Perceived Stress Scale (PSS-10), Kessler-6 (K6), and Brief Resilient Coping Scale (BRCS) to assess perceived stress, psychiatric morbidity, and resilience of the person. It had three parts, one to capture socio-demographic details of the participants including age, sex, marital status, occupation, family history of psychiatric morbidity, place of stay etc. Second part consisted of psychometric scales and third was designed to capture the views of participants on the coping strategies. Calculated sample size was 189. Result(s): Based on standard cut-off values, it was found that 65.6% subjects were under moderate or severe stress;56.6% had compromised mental well-being and 64% were not coping well with the pandemic situation. PSS were significantly poor for females(p-value<0.001),single(p-value<0.001)andthosewithout history of psychiatric morbidity (p-value <0.001) and low resilient copers (p<0.0001). Mental well-being was compromised more among married (p-value=0.01), doctors (p-value=0.008), aged <40 years (p-value=0.003), high resilient copers (p-value=0.02). Popular means of stress reliever were music and yoga/exercise. Correct and updated knowledge on disease transmission, availability of personal protective equipment, pursuing hobbies like music and gardening were few suggested measures to improve coping with stress associated with patient care. Conclusion(s): The study revealed that majority of the health workers experienced moderate to heavy degree of stress and compromised psychological well-being during the first wave of pandemic. Relationship of stress and psychological wellbeing with resilience and socio-demographic variables was not always linear. Copyright © 2022 Journal of Clinical and Diagnostic Research. BACKGROUND: The COVID-19 pandemic has harshly burdened the healthcare systems. Health care workers (HCWs) are at substantial risk of infection and confronted several stressors as well leading them to experience burnout. This study aimed to assess the prevalence of burnout among Lebanese health HCWs and to identify its associated factors. METHODS: A cross-sectional online survey was conducted between the first of November and the end of December 2020 among Lebanese HCWs working in all active hospitals operating across the country. Data were collected using an Arabic, anonymous, self-reported questionnaire comprising four sections: (a) basic sociodemographic characteristics, (b) exposure to COVID-19 covariates, (c) occupational factors, and (d) the measurements including the Copenhagen Burnout Inventory (CBI). CBI subscale cut-off score of 50 was used to assess the prevalence of burnout among HCWs. Multinomial logistic regression analyses were performed to examine the factors associated with the different aspects of burnout. RESULTS: Out of the 1751 respondents, personal burnout (PB) was detected in its moderate and high-level aspects among 86.3% of Lebanese HCWs. Moderate and high levels of work-related burnout (WB), and client-related burnout (CB) hit 79.2% and 83.3% of HCWs, respectively. HCWs who were females, married, physicians, having a poor health status and specific living conditions (dependent child, elderly at home, family member with comorbidities, and a low income) were more likely to exhibit a high level of PB compared to no/low burnout level. Moreover, frontline HCWs, those infected by COVID-19 or those having a colleague infected by COVID-19, and those exhibiting a high perception of threat were more prone to experience a high-level PB rather than a low/no burnout. Working in a public hospital, extensive working hours, and insufficient sleeping hours were also found positively associated with high PB. However, older age and altruism were negatively associated with high PB compared to no/low PB among HCWs. As for WB, similar factors were found either positively or negatively associated with a high level of WB except health status and living conditions factors (dependent child or family member). As for CB, older age of HCWs (> 30 years) and altruism were negatively associated with high CB compared to no/low burnout level. However, working in the frontline, high threat perception, extensive working hours, insufficient sleeping hours, and low income were positively associated with high CB among HCWs compared to no/low burnout. CONCLUSION: The prevalence of burnout among Lebanese HCWs during the pandemic was significant and alarming. Enacting and implementing preventive policies and effective interventions are urgently needed to cultivate wellness among HCWs.

ABSTRACT

Objective: This research aimed to assess the knowledge, attitude and practice toward COVID-19 among East Nusa Tenggara (NTT) population and identify associated sociodemographic factors. Methods: A cross-sectional study was performed among communities in 22 regencies of NTT between October-November 2021 using a structured and validated questionnaire that consists of 22-items of knowledge, 6-items of attitude and 13-items of practice related COVID-19 questions. A cut off point of at least 70% was employed to categorize good knowledge, attitude, and practice. Results: The percentages of survey respondents showing good knowledge, attitude and practice toward COVID-19 were 79.8, 72.7, and 94.6%, respectively. There was a significant positive, though weak, linear correlation between knowledge and practice scores (ρ = 0.097; p = 0.049). Knowledge was strongly associated with regency of residence (Cramer's V = 0.266; p = 0.010), education (Cramer's V = 0.312; p < 0.001), and occupation (Cramer's V = 0.313; p < 0.001). Attitude and practice had strong relationship with regency of residence (Cramer's V = 0.289; p = 0.024) and education (Cramer's V = 0.272; p < 0.001), respectively. Conclusion: Nearly all survey participants showed good precautionary behaviors, but there was still a quarter of respondents indicated poor knowledge and attitude. There was also a positive relationship between knowledge and practice scores thereby indicating the importance of local public health advocates to distribute information uniformly especially to the groups with inadequate knowledge toward COVID-19 as a means to control the virus transmission.

ABSTRACT

Background: Cannabis is one of the widely used drugs for relaxation and may be abuse among users. Researchers have given attention to cannabis use among the general population while vulnerable populations who have experience multiple traumas may be at risk of cannabis abuse. Other factors may influence cannabis abuse among people exposed to traumas. Objective: The study aims to determine the prevalence and correlates of self-reported cannabis abuse among residents of Fort McMurray, a city in Northern Alberta, Canada. Methods: A cross-sectional survey was conducted using an online questionnaire. Sociodemographic data, trauma exposure, and clinical characteristics were collected to identify the possible risk factor of cannabis abuse. Data were analyzed with SPSS version 25 using a chi-square test and binary logistic regression analysis. Correlation analysis was also performed to ascertain likely cannabis abuse and the association with other mental health conditions. Results: Overall, participants who completed the survey were one hundred and eighty-sixed out of the two hundred and forty-nine residents who accessed the online survey, giving a response rate of 74.7%. The prevalence of self-reported cannabis abuse among participants was 14%. Most of the participants were females (159, 85.5%), more than 40 years of age (98, 52.7%), employed (175, 94.1%), owned their house (145, 78.0%), and 103 (60.6%) reported being exposed to at least a trauma (COVID-19, flooding, or wildfire). Regarding regression analysis results, participants living in a rented accommodation were nearly four times more likely to abuse cannabis (OR = 3.86; 95% CI: 1.34-11.14) than those owning their houses. Similarly, male participants were more than 6-folds more likely to abuse cannabis than the female gender (OR= 6.25; 95% CI: 1.89-20), and participants in a relationship were six times more likely to abuse cannabis than participants not in a relationship (OR = 6.33; 95% CI: 1.67-24.39). The study also found significant association of depressive symptoms (X 2 = 4.561; p = 0.033) and anxiety symptoms (X 2 = 4.700; p = 0.030) with likely cannabis abuse. Conclusion: Demographic factors significantly predicted likely cannabis abuse in the Fort McMurray population, and cannabis abuse significantly correlated with presence of moderate to high anxiety and depression symptoms. It is essential to mobilized resources to support vulnerable communities and manage cannabis abuse.
